Sole custody of medical records

I haven’t done much to figure a plan of how to work this, but something needs to be done about the state of our medical records. We have no option to change false diagnosis, companies are being breeched and every bit of data medical, financial and personal are being extracted from companies/affiliates we never did business with and should not have all our info. We should have the option to put that stuff on a blockchain, dispute and change things that are incorrect or not relevant and extremely private, with no access unless you are given a revocable guest code. If someone could help formulate ideas on how this could work, that would be awesome.
