Universal Electronic Medical Record

I have been in private practice for 21 years. We desperately need a patient centered electronic medical record, which can be created by the patient, and then accessed by their various doctors, from their PCP to specialists, to ER doctors, where everything they receive is in the same place. This would virtually eliminate duplicative tests and greatly increase the efficiency of health care in this country.
