In the UK, a town called Frome innovated by creating an online database of every group in the community across all areas - health, social, specific hobbies, sports, etc. Then recruiting 2% of the local community as designated community connectors (hairdressers, taxi drivers, clinic health coaches, etc) to connect lonely people to the database and find them groups to join.
This lead to large reduction in healthcare costs and large scale community cohesion.
The downstream benefits are incredible across many measurable and immeasurable numbers
